Comment / Review | How did the scammer come to know we had been in close contact. Where did they get that information from? We cancelled our card the following day and so that number won’t be available any more.

Any communication from Medicare would be very professional and formal. The link the scammer included is a phishing link meant to steal your details.
As for your question about how they knew you were in contact with someone who tested positive- they didn't. They send these texts/emails to everyone because the pandemic is still ongoing and there is bound to be someone out there who really was in contact with a covid-positive person and they're banking on that to get people to click on the link and give out their bank account information to 'purchase PCR kit'.
