        Text message saying I’ve been in contact with someone with Covid and to link to nhs to request PCR test
        The page looked like every other NHS page I’ve seen
        I’ve tried to access the page again but it is no longer there

        Full name
        Address
        Date of birth
        Mobile telephone number
